NHS London has overall responsibility for the performance of 31 primary care trusts (PCTs) - who operate in five clusters, 16 acute trusts, three non-FT mental health trusts and the London Ambulance Service. A further 18 acute / specialise trusts in London are foundation Trusts and regulated by Monitor.

London created sectors in early 2010 as a cost saving initiative. Sectors are groups of PCTs that work together and have one management team.
The DH 2011/12 Operating Framework outlined the process for this happening nationwide by 1 April 2011 and announced that these groups of PCTs would be called clusters. National consistency is important so London is now adopting the term clusters. From now on London sectors should be referred to as clusters.
During the transition, the clusters will keep a tight grip on performance and quality in order to hand over a strong platform to GP consortia.
