Serious Untoward Incident (SUI) Reporting Guidance

NHS London needs to receive information on Serious Untoward Incidents (SUIs) from NHS organizations within its boundaries in order to both identify learning opportunities for improving patient safety, and to ensure that NHS Trusts and Primary Care Trusts (PCTs) have robust arrangements in place to investigate incidents and prevent reoccurrence.
