Report of the external review of John Meyer Ward following the death of Eshan Chattun On 17 June 2003

Eshan Chattun died after being attacked by Jason Cann, a patient on John Meyer Ward in Springfield Hospital Tooting where Eshan had been working as a health care assistant. Jason was arrested and charged with murder. He was convicted of manslaughter on the grounds of diminished responsibility.

South West London and St George's Mental Health NHS Trust held an internal inquiry in accordance with the Trust's Serious Untoward Incident Policy.

The South West London Strategic Health Authority decided after consultations with the families of Eshan Chattun and Jason Cann that an independent external review should be commissioned. The review would not be a formal independent inquiry, but would be undertaken in accordance with the recently amended Department of Health Guidance -HSG (94)27.
