In March 2005 John Barrett was sentenced for manslaughter by reason of diminished responsibility and - in accordance with National Health Service guidelines - an independent inquiry into his care and treatment was commissioned by the South West London Strategic Health Authority (the predecessor of NHS London).
The relevant guidelines require that whenever a person receiving support from psychiatric services commits a homicide there should be an independent inquiry to examine the appropriateness of the care provided, point to any lessons to be learned and make recommendations.
In the period before the manslaughter of Denis Finnegan, John Barrett had been receiving support from psychiatric and social services in South West London.
